First Year Forms

First Year Application
In the First Year Application, applicants complete basic information such as contact details, academic information, extracurricular activities, and coursework, among other important pieces of information. Many colleges also require a Personal Statement (essay) as part of the First Year Application.
Instructor Recommendation
The Instructor Recommendation should be completed by an academic instructor who knows the applicant well. The instructor will be asked to rate the applicant on certain academic and personality traits and optionally provide a written evaluation.
School Report
The School Report should be completed by a counselor or other individual familiar with the applicant's academic background and capable of also sending the high school transcript. The counselor will be asked to rate the applicant on certain academic and personality traits and optionally provide a written evaluation.
First Marking Period Report
The First Marking Period Report should be completed by a counselor after submission of the School Report. The counselor will have the option to include an updated transcript upon completion of the applicant's first marking period.
Midyear Report
The Midyear Report should be completed by a counselor after submission of the School Report. The counselor will have the option to include an updated transcript upon completion of the applicant's midyear marking period.
Final Report
The Final Report should be completed by a counselor after submission of the School Report. The counselor will have the option to include an updated transcript upon completion of the school year.
Arts Supplement
Applicants with a significant interest or talent in art and/or music and wish to provide samples may do so as part of the Arts Supplement.
Athletic Supplement
Applicants with areas of athletic interest may provide more detail such as positions played, letters earned, and specific accomplishments as part of the Athletic Supplement.
Early Decision Agreement
Under the Early Decision plan, an applicant may apply to only one college. The Early Decision Agreement allows the applicant, family, and counselor submit acknowledgement of these conditions to the college selected by the applicant under the Early Decision plan.

Financial Aid

Once you get into a great college you have to pay for it. Use these resources to begin learning more about how financial aid can help make college affordable for you. Be sure to visit the college's Financial Aid website for more specific details regarding financial aid eligibility and requirements.


Federal Student Aid for College or Career School

The U.S. Department of Education’s office of Federal Student Aid provides grants, loans, and work-study opportunities to eligible students attending participating colleges or career schools. Net Price Calculator Center

Net Price is the amount you will pay to attend college for one year after scholarships and grants are considered. Your Net Price may be different for each college and so be sure to visit their website and use their Net Price Calculator to see what students like you have paid to attend that particular college. Fee Waivers

Fee waivers are forms available to students who may not have the financial resources to pay examination fees or the application fee to apply to college.
